Request LSS Home Health As Your Provider

All individuals have the right to choose their home health provider.  When you are discharged from the hospital, a rehab unit, or learn you need home care during a doctor’s visit, you should be offered a choice of agencies.  As you consider your options, keep in mind LSS Home Health specializes in rehabilitative care for older adults. 

Ask your social worker, discharge planner or physician to call us with a referral.  The LSS Home Health staff will coordinate all the skilled care you need. Call LSS Home Health at (314) 963-3430 for more information.
